For any records to be released, a formal Release of Information form needs to be completed and signed by the patient or a guardian. This form should state what records are requested and where the records should be sent. Records should be sent to the facility (HPSC, MVSC, or Oakdale) where the services were provided.
Medical records requests are normally filled within seven to ten business days.
